What is a deductible?
A deductible is the amount of medical expenses that you must pay before the plan will begin to cover expenses. For example, if you select a $1000 deductible, you pay the first $1000 of your covered medical expenses before your health plan begins paying expenses. By selecting a higher deductible, you will lower your monthly premium.

What is coinsurance?
Coinsurance is the expense you share with the insurance company after you have met your deductible. For example, if you have a $1000 deductible and an 80/20% coinsurance plan up to $5,000, you would pay:

• The first $1000 of covered expenses as your deductible,
• 20% of the next $5,000 of covered expenses, or up to $1,000.
• Once your deductible and coinsurance are met, your covered expenses are paid at 100% for the rest of the year.

What is a PPO?
PPO stands for Preferred Provider Organization. You maximize your benefits by using providers which are part of our PPO network. Since PPO providers have already agreed to negotiated charges, they will bill you only for your deductible and coinsurance amounts based on the pre-set prices.

If I obtain treatment from a provider who is not in the PPO network, will my charges still be covered?
You always have the freedom to choose any provider you wish. However, your benefits will be reduced for providers who are not in network. That means you will pay more out-of-pocket for non-network providers. We have extensive networks throughout Georgia.

How long does it take to find out if I am approved for coverage?
In most cases, coverage can be approved in 48 hours. If you have a more detailed medical history, additional time may be required to review your application for coverage
